If you do not have a Login Account to the KDADS Oracle Web Applications, or if you have moved to a different organization/provider, you must complete the KDADS Web Application Access Security Agreement.

Enter the e-mail address that is associated with your KDADS Oracle Web Applications account.
Enter your Username. Only include the Employee ID if you are a KDADS employee, otherwise leave blank.
Begins with a letter (a-z or A-Z) or number (0-9)
Contains at least one lowercase letter (a-z)
Contains at least one uppercase letter (A-Z)
Contains at least one number (0-9)
Must be 12 to 20 characters long.
Contains at least one special character from the following: #$^+=!*()@%&
Cannot contain any spaces.
Contains no more than three repeated characters.
