Enter your Username. Only include the Employee ID if you are a KDADS employee, otherwise leave blank.
Begins with a letter (a-z or A-Z) or number (0-9)
Contains at least one lowercase letter (a-z)
Contains at least one uppercase letter (A-Z)
Contains at least one number (0-9)
Must be 12 to 20 characters long.
Contains at least one special character from the following: #$^+=!*()@%&
Cannot contain any spaces.
Contains no more than three repeated characters.
